He has a particular interest in conditions such as low back pain, sciatica, nerve root pain, neuropathic pain, complex regional pain syndrome, fibromyalgia, and persistent pain affecting the knee, hip, and shoulder. He also offers advanced interventional treatments such as radiofrequency denervation and frequently uses combined ultrasound and fluoroscopy (CUF) techniques to enhance the safety and effectiveness of procedures.

Dr Dhotarkar works closely with specialist pain physiotherapists, psychologists, and spinal surgeons, partnering with patients to develop individualised management plans that address both physical and psychological aspects of pain. He is also actively involved in transitional pain services, an evolving area of pain medicine in the UK aimed at preventing long-term post-operative pain.

Alongside his clinical work, Dr Dhotarkar is a faculty tutor for the Royal College and regularly teaches on national and international courses in ultrasound-guided pain interventions and regional anaesthetic techniques.
