Raf's route into the specialty was a long one. Oral and maxillofacial surgery is the only surgical discipline in the UK that requires dual qualification in both medicine and dentistry, and his training spanned nine years at university — completing degrees in dentistry, medicine, and an intercalated BSc — followed by postgraduate surgical training before appointment as a consultant. Alon g the way, he has been recognised with a number of awards and prizes at various stages of his training and consultant career, including for surgical technique, research, and clinical performance.
The largest part of his practice is the surgical treatment of mouth, tongue, jaw and salivary gland cancers of the head and neck, together with the reconstruction that follows. He performs over 100 free flap reconstructions per year across his NHS and private lists — fibula, anterolateral thigh (ALT), radial forearm, and scapular tip flaps — which places his series among the highest-volume microsurgical head and neck practices in the UK. This matters clinically: outcomes in complex oncological resection and reconstruction correlate strongly with surgeon and unit volume, and Raf's private patients have access to the same high-volume expertise that underpins his NHS work.
He has particular subspecialty interests in microsurgical nerve repair (lingual and inferior alveolar nerves), salvage and revision surgery after previous treatment elsewhere, and reconstruction of the maxilla and midface using vascularised bone — an area where he advocates strongly for free flap reconstruction over traditional obturator prostheses where appropriate.
Parotid and submandibular gland surgery forms a further significant part of his practice, both for malignant disease (including primary salivary gland cancers and metastatic skin cancer within the parotid) and for benign conditions such as pleomorphic adenomas, Warthin's tumours, and chronic sialadenitis. His approach to salivary gland surgery emphasises facial nerve preservation, meticulous dissection, and the use of modified or extended facelift-style incisions where oncologically safe, so that the scar sits discreetly behind the ear and along the hairline rather than across the side of the neck.
Alongside his cancer and reconstructive work, Raf continues to treat the full spectrum of general oral and maxillofacial conditions: surgical removal of wisdom teeth and other impacted teeth, dentoalveolar surgery, jaw cysts and benign odontogenic tumours, facial and dental trauma, temporomandibular joint (TMJ) disorders, salivary duct stones and obstruction, oral mucosal disease, oral dysplasia and premalignant lesions, and benign lumps and bumps of the face, lips, and mouth. He also accepts referrals for second opinions and for management of post-treatment complications including osteoradionecrosis and medication-related osteonecrosis of the jaw (MRONJ).
Two principles shape the way he operates. The first is a commitment to the smallest, best-placed scars the operation allows. Incisions are designed to sit within natural skin creases, behind the hairline, or in the shadow lines of the face and neck, and this attention to cosmetic outcome is pursued alongside — never at the expense of — complete and accurate tumour excision with clear margins. The second is a genuinely patient-centred consultation. A suspected or confirmed cancer diagnosis is one of the most difficult things a person can face, and Raf believes patients deserve unhurried time, plain-language explanations, and a treatment plan they actually understand and agree with. He is known for being approachable, uses his first name with patients and junior colleagues alike, and is comfortable consulting in English or Polish.
His guiding clinical philosophy — "your first chance is your best chance" — reflects the evidence that outcomes in head and neck cancer are heavily dependent on getting the initial surgery right. Patients, GPs, dentists, and specialist colleagues are welcome to refer directly for suspected malignancy, a new diagnosis, a second opinion, or management of a complex benign condition.
