Sein Medizinstudium absolvierte er von 1992 bis 1999 an der Freien Universität und Humboldt Universität zu Berlin, Buffalo (USA), Bern (Schweiz). Im Laufe seiner Karriere war Priv. Doz. Dr. med. Dietmar Jacob in mehreren Krankenhäusern und Kliniken Oberarzt für Viscerale- und Gefäßchirurgie. Im Jahr 2014 erfolgte seine Niederlassung im „Chirurgischen Orthopädischen Praxisverbund" (COPV). die Gründung des hernienzentrum.berlin und der proktologen.berlin. Im Jahr 2021 erhielt er das Ehrendiplom als Europäischer Hernienchirurg: Fellow oft he European Board for Surgery Abdominal Wall Surgery (F.E.B.S. AWS).
Doktor Jacob ist als erfahrener Allgemein- und Bauchchirurg auf die operative Behandlung von Bauchwand- und Leistenbrüchen spezialisiert. Neben den herkömmlichen konventionellen Verfahren liegt sein Fokus in der Minimal Invasiven Chirurgie (MIC), um dem Patienten eine gewebeschonende und damit schmerzreduzierte Operation zu ermöglichen.
Ein weiteres Tätigkeitsfeld als Chirurg mit der Zusatzbezeichnung Proktologie liegt in der Behandlung von Leiden und Erkrankungen um den Schließmuskel sowie des Mastdarms wie z.B. das Hämorrhoidalleiden. Dabei werden nach Möglichkeit neueste Verfahren entsprechend den gegenwärtigen Leitlinien angewandt und zum Zwecke der schnelleren Wundheilung auch plastische Verfahren angewandt.
Im Rahmen seiner Lehr- und Dozententätigkeit nimmt Doktor Jacob an wichtigen nationalen und internationalen Kongressen Teil und ermöglicht damit seinen Patienten den neusten Stand der Wissenschaft anzubieten. Im Rahmen der Qualitätssicherung nimmt der COPV an der Qualitätssicherungsstudie Herniamed freiwillig teil. Dabei wird bei allen operierten Patienten mit Bauchwand- und Leistenbrüchen eine Nachsorge nach 1, 5 und 10 Jahren durchgeführt.
